That is a "Timing Mode Input Board". It plugs into any slot in the Apple II and the cable connects to the "Access Socket" on the 3-channel synthesizer card (originally "Apple Music Synthesizer" later "Music Card MC16"). The "Access Socket" would be the empty DIP socket at the upper right of the card (the card marked 5-1-16A on the circuit side). It allows one channel of the synthesizer to be used to control the song playback speed (tempo), instead of using the Apple II's game paddle (which is actually a variable timer).
