In the Slots Control panel slots 5&6 are normally set to Smart Port &
Disk Port while Startup is usually set to Scan.
When you have a setting in the Control Panel set to the factory
default there will be a check mark to the left of that particular setting.

If you hold down the OPTION key while powering up your IIgs or
pressing together OPTION CONTROL and RESET you'll get a menu
which will allow you the choice of setting the control panel to factory
standards.
