Slot 5, Drive 1 is right for your first 3.5" drive.
Just ProDOS does not include a way to format or initialize diskettes.
To create a bootable ProDOS disk you can Initialize a diskette from the
Finder and copy a few files. You want to copy BASIC.SYSTEM and, from the
System/ folder, P8. On your diskette, rename P8 to "PRODOS".
