Could be a lot of things including a bad drive but a few other causes of I/O
errors come to mind.
Covered write protect notch - notch must be uncovered and toward the left
hand side of the drive
Damaged write-protect switch
Using High density disks
Dirty drive heads
If you have two drives you might try swapping the connectors on the Disk II
card
