On the DuoDisk "boot trashing" problem...

I've experienced this.  A trigger that can set it off from DOS 3.3...

(this is from a long-repressed memory, so it may not be entirely correct)

]INIT filename,S6,D2

...then while the INIT process is going on, hit Apple-RESET.  The disk in
Drive 1 will start to boot, then shift to formatting in mid-boot.  Whoops!

Another trigger has to do with restarting during the Apple PASCAL boot
process; for instance while booting a PFS:File program disk (that was how
I managed to kill a PFS:File program disk...).

It's been...probably fifteen years since I had to deal with this, but I
remember that it was a rare situation, but most annoying when it happened.
