First guess would be the power supply -- the 5V line may not be
regulated properly anymore.  The problem with removing and
re-inserting cards is it tends to short out the supply.  I've blown
the P5A/P6A PROMS that way, but since yours seems to work some of the
time, they're probably all right.   If it's not the power supply, it's
probably the aforementioned PROMs, though I haven't heard of them
failing intermittently.
