Just a short note: I'm working on this.  My good ol' Apple II is
situated out in my country house, which I visit irregularly -- right
now it will be unavailable to me for a few weeks.  I do have .DSK
files though, containing the binary file ADT.CC (where CC means
"Communications Card"), however these .DSK files won't be of much use
for someone who has no working ADT on his Apple.
 
So I'm currently writing a utility to manipulate .DSK image files,
and to, among other things, extract any file within the .DSK image
and convert it to some suitable form -- for instance a monitor dump
of the Apple II.  THis dump could be transferred as a text file to
your Apple (if you have ASCII Express or some equivalent program), or
if you have nothing, you can type it in manually.
 
Expect it to be posted here within a few days.
 
Later I will make some decent assembly source files available too.
They're currently broken, so I have to recreate them.
