Go to http://www.grin.net/~cturley/gsezine/GS.WorldView/v2000/Mar/MiscNew/
and scroll down until you find dos33dmp.zip and dos33dmpinfo.txt. These
two files will allow you to create a working DOS 3.3 boot disk for your
Apple II+. You'll need to create a link between your PC and the Apple II+
with a serial card.

Nope. PC drives and Apple drives use different encoding schemes for
storing data on floppy disks.

Yes, this will work, if you want ProDOS. DOS 3.3 doesn't work with 3.5
inch floppy disks. There was a version of DOS that could use 400K disks,
but no more than that.
You will need to get a card. If you want a UniDisk drive, you'll need to
get a UniDisk controller for it. Most other 3.5 drives can be connected to
a card called the UDC (Universal Disk Controller). Whatever card you
decide to use will go in slot 5.

Yes, this would work. You'll need to do this if you want to get DOS 3.3 on
a disk using the dos33dmp files.
Yes, you'll need a card. There are several serial cards you can use. The
most common one is called a Super Serial Card. If you go onto eBay, you
might be able to find a serial card (or, better yet, an Apple IIgs, which
can run just about everything an Apple II+ can and has built-in serial,
mouse, and disk "cards," a faster and more advanced processor, and better
sound and graphics).
