My MS Z-80 card also sits in slots 4.  It has one dipswitch labeled SW1 in
roughly the center of the card, with 4 switches.  The first (left most) is
off (lower part towards connector depressed), and the remaining three are
set to on.  Honestly, I don't know what the values of these switches are,
since I bought the card used, but this is the setting I've used for years.
Frankly, the card's a bit flakey on the IIgs, but it's run flawlessly for
years on my Apple ][ and Apple ][+.
