Sounds like you are descibing 'The Visible Computer: 6502'.  The company that 
wrote it also did versions for other platforms; I have the 8088 version at 
work.

I think that I still have a copy of the 6502 diskette (but no the book).  I'd 
have to did thru my box of Apple // diskettes to verify that though.
