This sort of thing used to happen on our II+ (and, it crops up on other
machines). A fairly common explanation is a poorly seated KB encoder IC or KB
ROM (when there is one) or ribbon connector (when the KB circuit is mounted to
the KB). A faulty KB encoder IC or ROM is a possibility, too; but, a poor
socket contact is more likely.
A try at fixing things is to scootch up and re-socket the KB encoder and
KB ROM in order to reestablish socket contacts (assuming the IC's are in
sockets).
