Digital Prism cable

I've got an Applied Engineering Digital Prism RGB card in 
my IIe. I'd like to use it with an Apple IIgs RGB monitor 
but I lack the cable between the card and the monitor cable. 
Could anyone help me with the pinout for this cable.
