    There was an official Centipede port by Atari, and as far as 
I can remember, probably a half dozen clones going by different 
names (Bug Attack and Night Crawler come to mind). I don't think
Milipede was ever ported to the Apple II.

    Yep, there is an excellent port floating around, though it 
was never commercially released. Jimmy Huey wrote this port 
back in early 80's (he was lurking around comp.sys.apple2 not
long ago as I bumped into him here and chatted with him).

    No, I don't believe so. However there were ports of varient
games released for the Apple II, like Ikari Warriors I and II,
and Heavy Barrel. I personally prefer them under MAME. :)
