If there is such a document, it was never released publically, or I
would have got it as fast as I could.

It _might_ have been made available to attendees at some developer
conference (for example), or only been available to developers who
signed a non-disclosure agreement.

I expect it was just educated guesswork.  I haven't looked at what
ORCA/Disassembler produces, but I wouldn't be surprised if the header is
similar to the one used in GS/OS device drivers, which are very well
documented.
