This is Ultima IV: Quest of the Avatar for the Apple II series of computers. 
One of the best role playing games of all time. Included is the box, two 5.25" 
disks, cloth map of Britannia, The History of Britannia, The Book of Mystic 
Wisdom, and the Player Reference Card.
