Sounds to me like you have one of a couple of prolems:

First, it could be a bad boot disk. Try a different disk. Try an old game
and see if that boots properly.

Second, a ][+ won't boot a ProDos disk. You need Dos 3.3, or alternately,
the old Apple Pascal disk system. Check the label on the disk.
