it's no surprise if it doesn't work on a IIgs ROM 01 (even at 1 mHz)?
Or should I be concerned that the card is defective?

It's not a surprise that it won't work reliably on a IIgs. Some of them won't even work reliably on a IIe - or, as I implied earlier, might only work on _some_ IIes.
