Multiflex Modem Info Needed

I have a Multiflex Modem Ver 2.1 card for an Apple ][ that I would like to 
get running.  I realize this is an antique modem, but I like restoring 
antique computer equipment (to my wife's dismay).

I need some information:

     1.  What chip should be in U11?
     2.  What is JP1 used for?
     3.  Why is there a choice of LEFT or RIGHT phone connectors?
     4.  Was any special software required?

I promise to keep the identity of anyone who provides help on the above from 
my wife.  :)
