ID this card (also for sale)

I have a card that has the following characteristics:

	4 chips, 1 of the them being a Rockwell R6522P (R6522-31)
	4 RCA jacks on it labeled:
		J1 Monitor
		J2 Disc Video
		J3 Apple
		J4 Disc Control
	Stencil in top left: "Video Disc Controller"
	Stencil in bottom left: "Copyright 1986 Whitney Educational Services,
		San Mateo, CA 94402"
	Stencil in center left: (note: large, fancy script graphic) "Hoffman"

The card looks brand new (as though it was just made).  I assume that it is
just as it says, a video disc controller, probably for video editing in a
classroom or something like it.

sale/trade.
