The *7004 ... indicates that program execution is going somewhere it
should not and bumping into a Break instruction. You may have a bad IC
or an IC or connector or Slot card which is not making good contact on all
pins.
There are all sorts of tests you could do; but, it may be easier to
try the 'usual cure' first. With power OFF, scootch up and resocket each
IC; and, unplug and replug each internal connector and Slot card.
