   Those vulcan drives tended to flake out a lot, especially this many
years after their manufacture. I'd recommend booting off another valid
ProDOS disk, and verifying the integrity of the vulcan drive. One of
the best ProDOS verification programs is ProSEL 8, sold by Charlie's
Appleseeds. The following contact information for them is taken from
the comp.sys.apple2 Frequently Asked Questions (FAQ) at

   which includes disk fixing utilities for ProDOS disks.

   I'd seriously recommend verifying the disk integrity before
proceeding, but if you feel that the problem is not serious (such as
accidentally deleting that file), the just copy the missing files back
to the HD.
