This is just a standard error which occurs with the combination
of disk access and system crashes.  The same corruption occurs
on ProDOS disks.  However, since there are ProDOS volume disk
checkers, editors, and repairers, you can correct this problem
without a complete reformat.

The "bug fix" would be to write a disk correction utility which
can manage HFS partitions for the IIgs (they already exist for
the Mac!).
