   According to the list of error numbers at
http://www.visi.com/~nathan/a2/faq/gserrors.html (which is referenced
from the comp.sys.apple2 Frequently Asked Questions (FAQ) at
http://www.visi.com/~nathan/a2/faq/csa2.html) error $4B is "OS:
Unsupported storage type." 

   Not sure of the exact cause in your case, but I suspect the file or
disk has gone bad, and the system can't read it reliably. The other
probable cause is that you're booting a System disk too old to support
what Ultima I wants to do (I think it may be a System 6.x program, but
I'm unsure of that). What version of GS/OS (if it says ProDOS 16, it's
definitely too old) are you booting? Hit space at the start of the
GS/OS graphical boot to have it identify itself if it's not readily
apparent.
