From the comp.sys.apple2 Frequently Asked Questions at
http://www.visi.com/~nathan/a2/faq/csa2.html section 8.5:

--- Snip

8.5 I'm getting Error XXXX or YY. What's it mean?

A: Some common errors and their cause:

A larger list of all error messages, but not always the best
description of the cause is at
   http://www.visi.com/~nathan/a2/faq/gserrors.html.

[snip]
GS/OS errors:
[snip]
11xx GS/OS could not successfully load an application or program
---

In practice, this tends to mean that the app is corrupted on disk.
If you can load it find from some launchers but not others, then I'd
suspect memory trashing corrupting the file as it's being
loaded/relocated. Try a reinstall from the master disks.
