    Years ago, what I would do in this case, was to take a second
5.25" floppy disk and flip it over on to the first. Then, using a
pencil or pen, carefully draw an outline of the new notch by using
the existing notch as a guide. That way there was no question of 
cutting the notch too deep or in the wrong space. After that I'd 
use a use a pair of nail-scissors to make three snips, ending up
with a nice clean (usually :) square notch.

    In later years I bought a disk-notcher that did all the work
for me (and made a notch as clean as pre-punched factory notches),
in fact I still have it today. Of course I rarely use it, or 5.25" 
disks at all these days, but it still comes in handy from time to
time. :)
