I saw that failure quite often on the unenhanced IIe if the self-test
was run immediately after certain other software (but I can't remember
which software - this would have been in 1983 or 1984). Rebooting with
Ctrl-Apple-Reset then running the self test again usually cleared it.
It appears that the original firmware's self test doesn't set up some
soft-switch properly. If it has been left in an unusual state by
previous software, the self test thinks there is a problem.
I've never seen this problem reported on an enhanced IIe, so they
presumably fixed the self test.
