IWII startup problems

ImageWriterII problems:
I am trying to active an ImageWriterII from start.
Problem 1: the error light keeps blinking. The front cover is
well seated. Cartridge is well seated.
Problem 2: The select light never comes on.
Problem 3: Line feed/form feed dont feed.
Problem 4: Selftest doesnt work.
Causes? Solutions?
