First guess is that while the directory area of the disk is readable, one
or more sectors occupied by files are not. Hence, during the boot
process, an error is encountered.
I suggest that you (re-)check the disk on a PC, running Scandisk or some
other "Is this disk OK?" util against it. You may need to repair the
disk, format it, or even use another disk.
In general, a 'standard' boot-able "normal density" MS-DOS disk will work
just fine. From scratch, you can create such a disk on a PC by formatting
it and using the /S switch to assure that the 'system' files are added.
Depending on the configuration of the PC drive that you use to format the
disk, you may have to assure that the disk is formatted at 'normal' (360K
for a 5-1/4" disk) density, not 'high (1.2 MB) density.
