Help in I.D'ing a card...

I 've gotten a hold of what lokks like an old sound card from the Apple II+
era. I was wondering if anyone can help me I.D. it...

The card has  OAK written on it, it has a small speaker attached and it 
contains the following I.C.s

74LS273 8312A0
MC1408L8 8219
MT6008

U-Com (I assume this means Microcom) is inscribed on the back of the board.

Any help identifying it and possibly finding some software to use with 
it would be appreciated.....
