Help copying King Cribbage

I need help making a copy of an old Apple II game.

I've got a copy of King Cribbage which I have been playing for many
years now.  However, the disk is physically starting to degrade, so I'd
like to make a copy of it.  The problem being though, is that I can't
get a working copy  due to the current copy protection.

Does anyone out there know the parms in Copy II+ for making a copy of
this disk:
  King Cribbage by Hayden Book Co., published in 1981 (on 5 1/4 of
course).

Any help with this would be greatly appreciated.
