There are three banks of DIPs with 8 switches each.
 
Set 1 (far left) controls the language settings. 
   American (default) is open-open-open.  
   Now comes form length with 11" being open (default), and 12" being closed.  
   Next comes performation skip: inactive is open (default): and active is
closed.  
   Character pitch is next: 10 CPI is open-open; 12 CPI is closed-open
(default).  
   Proportional-pitch printing requires both be closed.  
The last swith (#8) in number controls the carriage return.  CR is open
(default), CR plus line feed is closed. 
 
   Set 2, in the middle, controls comm speed and paper handling.  
A comm rate of 19,200 is open-open (default); 9600 is closed-closed
Protocol is next.  Hardware handshake is open (default);  XON/XOFF is closed
Got an option card installed?  No card is open, a card installed is closed
Got a cut sheet feeder?  
    One bin or one bin and an envelope feeder attached is open-open-open
    Two bins or two and an envelope feeder attached is closed-open-closed
    Three bins or three and one envelope feeder is (open or
closed)-closed-closed
The last switch controls auto paper loading.  
     To load one-sixteenth above the print line is open (default).  
     To load one and one-sixteenth above the print line (used with the pull
tractor) is closed.
 
   Do NOT adjust the third DIP switch block.

    Have you checked for hidden printer codes around the 5th page in what you
want to print?
