Everytime I've seen that problem it has been because the drive was
faulty. I have an Apple //gs, a Macintosh Plus and an IBM clone with a
SCSI host adapter. Any drive that ever gave me those results did one of 2
things. A: was not recognized during format on any machine, or B: behaved
properly after being formated with the Apple using the correct ID #. Most
of the time the results were A:
