Your 16k board should be in Slot #0.  I'm guessing it was right to begin with.

There may be a little 16-pin ribbon cable plugged into one of the board
chip sockets.

Have you checked the offending ProDOS disk on another A][?  It seems
possible that you have a bad system disk.  I've gotten those errors on my
][e with ProDOS.

As for a warm boot, (actually, the 3-finger salute on the ][e is a cold
boot), you have a few options:

a.  Press {RESET} to stop everything, and you should land at an Applesoft
prompt (]).  Type PR#6 (actually, PR#slot with disk controller) and you
should be in business.

b.  Do the above without pressing RESET if you're already at a BASIC prompt.

c.  From the Monitor (prompt is "*"), type one of the following:

*C600G
*6 ^P  <-that's "CTRL-P"
*^B    <-"CTRL-B". you'll get the Applesoft prompt after you press RETURN
]PR#6

That's about it.  If you want to get creative, I suppose you could rig up
a switch in the power supply line that would interrupt power to the ][+
when pressed.  8-)
