I have a slightly different revision of that card here. Mine has five plate-through holes in the upper right instead of the DIP socket. They are labeled, top to bottom:

RED
GREEN
BLUE
OV (Gnd)
SYNC

There is bundle of wires about 10" long soldered in there, with a DB-25M on the other end:

Pin 2 ---
	|--Sync
Pin 4 ---
Pin 5 --- Green
Pin 6 --- Blue
Pin 7 --- Red

I have no clue what type of monitor connected to this. The bottom right of the board has three holes:

Vid
GR

A short piece of ribbon cable brings these out to a pair of machine-pin DIP sockets that are clearly meant to sandwich in between two of the motherboard ICs and their sockets.

Never been able to find a shred of information on this card.
