Aztec C can create very decent code but this often depends on the programmer. You might also want to examine the HyperC environment. It's another K&R compiler system. There's quite a bit of code and doc's floating around for it also.
It seems folks here tend to forget the options for BASIC. The Beagle compiler for Applesoft is freely available and usable. It is reasonably transparent for the end user and doesn't hinder development too much. Couple that with their program editor and one can create some pretty good software.
Alternately, there are BASIC compiler environments such as ZBASIC (which I personally used back when and quite like) as well as Micol advanced BASIC. Both of these are freely available and documentation are online. These are two factors I and I suspect others, find important.
There are also options for DOS 3.3 Applesoft and Integer. See Flash compiler for example. Also with documentation.
