It is called "System 6.0.1".  GS/OS is the operating system, which is
only one component of the system software.  GS/OS has a separate
version number, and is version 4.0.3 on System 6.0.1.

Quality Computers used to sell it with some bonus software, but I
can't see it mentioned in their latest "Enhance" catalogue or a recent
issue of "II Alive".

ByteWorks doesn't appear to sell it.  I would have expected them
to have the APDA version (programmer's, without a manual).

Some companies include a copy of the system software with other
products.  This is often a minimal system disk, and the other five
disks in the set are not included.

Do you mean you are having trouble using FTP, or do you not have an
easy way to get the files to an Apple II?

Make that six disks.  At least three are required (Install,
SystemTools1, SystemTools2).  Two more have optional system software
components (SynthLab and Fonts).  The last is a pre-installed System
Disk, which can be created using the Installer.

Note that you must have at least 1 megabyte to use System 5.0.4 or later.

Yes to both.  The best option is probably Sequential Systems' "RAM-GS"
card, which is only available with 4MB, and is probably only directly
available from Sequential Systems.  At last count, it cost something
in the range US$120 to US$130.

Boy, it is hard finding their contact details.  They never have an
address or anything other than an 800-number in their advertisements
(not much use to me, as I can't ring US 800 numbers).

I finally found their address in an older issue of GS+ magazine, in a
product review.
