Extended SoundSmith already exists in a standard form, via the MegaTracker
driver found in SSPlay, Instant Access 2.2 and later, and other cool stuff.
All the effects are the same as MOD, except 3 and C are switched around,
and the pitch f/x modify the raw DOC frequency value instead of periods.
Also, due to DOC restrictons effect 9 is out.
