Are the top and bottom of the screen trying to jump inward simultaneously?
Does giving the monitor a sharp rap on the side sometimes cure it?  If so,
run, don't walk, to your nearest electronics repair shop, and tell them you
need a new flyback transformer.

My monitor had these symptoms once--the top and bottom of the screen kept
trying to jump simultaneously toward the middle, and sometimes the whole
screen went black.  The power light went off, but I could tell it was still
getting power because the "CRT whine" kept going (in fact it got louder).
A sharp rap on the side usually cured it.  I ignored the problem, because I
could usually cure it by hitting the monitor, but one day it refused to
turn on at all, and no amount of violence would make it work.  My friendly
local techie-type person informed me that the flyback transformer had blown
out, and that repairing the resulting damange would cost more than the
monitor was worth.  I had to get a new monitor.

Apparently, if the problem is repaired while the you can still see stuff on
the screen, it's a relatively cheap repair...you just need to fix the
flyback transformer.  If you wait until it blows out, it takes lots of
other parts with it, including some custom IC's, and repairing it isn't
worth the expense.
