Well, I went through similar problems recently. (added a tape drive to my
system). My solution was to shorten the total length of SCSI cable used.
I also made sure the cable was of high quality.

Oh, I also made sure that my tape drive had a SCSI ID smaller than my hard
drive. But, you're set up this way already. You may want to try to switch
the order of IDs anyway. Also, make sure that you have proper termination.
Only the last device in the SCSI chain should be terminated.
