Have you tested the board for bad chips?  That may be why it is crashing.
Also, don't put it in slot 3, since that is the slot used for the
80-column display, and don't put it in any slots that are mapped as hard
drive partitions (such as with the RAMfast or Apple SCSI card with more
than two partitions).
