LE patch for HardPressed (and other //gs) users on cba2

I've put a patch up on cba2 that provides a workaround for bugs in some
applications that called LEStartUp() with the arguments backward.  This
was written by Dave Lyons back in November '87.

The reason I'm posting it is that having both HardPressed and Pointless
in the system at the same time causes the direct page space to be set up
*just* *right* so that stuff like this freeware Yahtzee game I have will
crash if you go through the setup menu.

Somebody said it fixed a difficulty with Mean 18 as well; dunno 'bout that.

Gory details are in the cba2 posting and in the enclosed doc file.  This
version has a really minor (but Dave-approved) bug fix in it as well.
