need Prometheus card info

I have and apple //e which my wife just converted to a midi
sequencer. Problem is that the midi card really wants to be
in slot 2 where my Prometheus card (serial, parallel, clock, and
bsr) used to reside. The Prometheus card can be re-configured with
bank switches but I can't find the manual :^(  Does anyone have
this card (+manual) or know of a good source? I have the box the card
came in, called the phone number on the box and got mel's hair
salon or something. Is this company still around?
