You may be missing the SOS boot block, block 1.

If a ProDOS disk is formatted with some utilities, it will not have
the SOS boot block.  The ProDOS /SYSTEM disk has the SOS boot block,
but formatting a disk with the system utilities won't help if you
write over all the blocks using ADT.

It would seem that the SOS boot disk image should have the SOS boot
block, but it may not if it has been copied to a ProDOS disk without
the block by using file copy.

You can use a block editor (like Prosel's Block Warden) to copy
block 1 from a disk known to have the SOS boot block (like a ProDOS
/SYSTEM disk) to your SOS boot disk.
