I suspect that you will need a null-modem cable. Easy way to test:
1) On PC, run HyperTerminal and configure it for the COM port and baud
rate you want to use (set Flow control to None).
2) On Apple III in II mode, PR#n where n is the serial card slot.
3) Type on Apple III keyboard and watch for characters appearing in
HyperTerminal window.
This was how I tested my homemade cable for my IIc to PC connection.
