Digi-Key can make cable assemblies like this. Check out page 29 of
their catalog: http://dkc3.digikey.com/PDF/T072/P0029.pdf

They've got some standard configurations but they will also make
custom lengths. Otherwise, you can get a roll of ribbon cable and some
connectors and crimp them yourself.
