A digital camera, a tripod, and some good full-spectrum lights works alright
for me.  A few scripts to prepare the images and turn them into a PDF. 
About an hour for a 200-page spiral-bound book.  You have to have a camera
that lets you fix the aperature and shutter speed, however.
