If you search csa2 in Google groups for CCS7811 you
will find three threads where this card was discussed.
Search for 9511 and you will find more.  The manual
is posted in the csa2 Gmail account; you can see if it
matches what you have.  The patched Applesoft may have
been provided as a replacement E8 ROM.  Does the Apple
II+ have a non-Apple ROM?  That ROM may also be
installed in a CCS 7114 ROM card in slot 0.
