I don't think anyone has imagined that they could get a moder video card to output an authentic Apple II video signal. All the effort has been directed toward making a hi-res RGB display *look like* an NTSC monitor displaying the picture. This is hard enough to get right, and apparently has been seldom accomplished.
Some emulators do this now - notably NES/SNES ones - using Blargg's NTSC Libraries: http://www.slack.net/~ant/libs/ntsc.html There are some better comparison images here: http://www.io.com/~greens/ntsc-vs-palette/ If you look closely, you can see the rightmost images are quite reminiscent of double-hires on the IIGS RGB monitor, and the leftmost images look much more like what you'd see on a TV. I was planning to look into this, but Real Life has taken over. :-(
