SSC to Imagewriter ][ & Apple ][c to ImageWriter ][ cable

Hi Guys
Just wondering anyone could help me out on the pin connections from the SSC 
to Imagewriter ][ (DIN 8)  and Apple ][c Printer port to ImageWriter ][ ?

I tried the FAQ's but nothing on Imagewriter ][ to Apple ][c & SSC.
I'm also having problems with my Apple ][c 9"mono monitor, after switching 
on for about 10mins the display just start to disappear intermittenly(gets 
dimmer & dimmer and finally blank). Any ideas which component is causing 
this?
