When I was using either a performa or LCII to format an SCSI drive I used a 
patched version of the Apple utility that didn't look just for Apple drives. 
It was found with a google search and worked fine setting up 2 prodos 32 meg 
partitions and the remainder HFS.

I didn't use this method ,I found a patched version of whatever I used.

If you don't have any luck let me know and I'll do a more thorough search of 
my drives for the software. A quick search didn't yield an obvious answer 
but I've got stuff spread all over the place.
